Medical records

There may be times when you need to release or request your full medical records. Patients may wish to release their UT Medicine medical records to another provider or they may need to request their medical records from another provider be sent to UT Medicine. In either case, we are here to help. To initiate this process, please follow the instructions below.

You can also view your medical record online through your UT Medicine patient portal.

The fastest way to release your UT Medicine medical record to another party is through the UT Medicine patient portal, powered by MyChart.

Alternatively, to request that a copy of your UT Medicine medical record be sent to another party, please print, complete, and fax a Release of Information Form (also available in Spanish) to 1-512-495-5680.

A patient or their legal representative can complete the Release of Information Form. Completion of this form allows UT Medicine to transfer the medical record in compliance with the requirements for protection of patient healthcare information (HIPAA). To request a copy of your medical record from an outside provider, such as a referring doctor, be sent to UT Medicine, please complete the Authorization to Receive Records Form and (also available in Spanish) give it to your current provider so that they can send your medical records to UT Medicine.

A patient or their legal representative can complete the Authorization to Receive Records Form. Completion of this form allows UT Health Austin to receive the medical record in compliance with the requirements for protection of patient healthcare information (HIPAA). Non-health care organizations — including law firms, insurance companies, and government agencies — can securely request medical records through a dedicated portal.
